Job Title:         Medical Records Clerk (Full Time)
Reports To:     Director of Nursing

Summary: The Medical Records Clerk shall possess general clerical skills including typing, filing, and inventory control skills.  Shall possess high standards of accuracy.  The employee responsible for the medical record system is either certified or trained to perform Medical Record duties and her/his work receives regular monitoring by a Medical Records Consultant and the Medical Records Supervisor.  The Medical Records Clerk is responsible to the Medical Records Supervisor.  Under the guidance of a medical record consultant, the Medical Records Clerk maintains the record-keeping practices of the facility as established by the facility policies and procedures.

Essential Duties and Responsibilities include the following. Other duties may be assigned.
- Maintain permanent register of residents - resident index.
-   Maintain disease index/coding in Registration Program and on cummulatives.
- Review records of discharge and admission charts using the chart audit.
-   Do Admission / 72 hr. / 7-day audit on all new admits and notification of Medical Records Supervisor of any missing items.
- Re-audit # 9 monthly for completions.
-   Thin charts on a regular basis.
-    Preserve and protect medical records in your custody.
-    Handle routine requests for medical information. (In house)

Job Title:         Administrative Assistant (Part Time)

Reports To:     Skilled Nursing Facility (SNF) Administrator

Summary - This part-time position will be under the general supervision of the SNF Administrator, and serve as the primary point of internal and external contact, performs receptionist and secretarial duties, and many aspects of the admissions process.  This position is responsible for assisting the SNF Administrator in ensuring SNF operations remain in compliancy with regulations, policies and procedures.

Essential Duties and Responsibilities include the following: (Other duties may be assigned.)

·         Serves as the receptionist for the Skilled Nursing Facility.
·         Greets and acknowledges visitors. 
·         Monitors the visitor and vendor badge program.
·         Receives and screens telephone communications.
·         Maintains an organized desk area, ensuring residents’ protected health information is kept secured.
·         Maintains SNF Conference Room and Work Room in order.  Oversees schedule for Conference Room.
·         Receives and distributes incoming mail.
·         Maintains the filing system for the office including the residents’ business files.
·         Monitors the usage, ordering and receiving of office supplies for the facility.
·         Provides clerical work for the SNF Administrator and other administrative staff.
·         Appropriately answers questions from admission inquiries.  Documents and tracks admission
inquiries.  Documents reasons for refusals.
·         Provides tours of the Skilled Nursing Facility.
·         Maintains a sufficient supply of Applications and Admissions Packets, ensuring documents are
current.

Computer Skills: To perform this job successfully, an individual should have knowledge of Database software; Internet software; Excel Spreadsheet software and Word Processing software.  Must have a working knowledge of electronic health records. 

Certificates, Licenses, Registrations - Must be able to type at least 60 words per minute.

Other Qualifications: Must demonstrate a professional manner in dealing with both the general public and the residents, have exceptional communication skills, be skilled at solving problems, and be capable of generating confidence in interpersonal relationships.  A working knowledge of electronic health records and a basic understanding of business procedures are also required, together with a total commitment to the Mission Statement of Bethany Home.

Assisted Living and Memory Care Facility

Part Time AM/PM Shifts Available

Bethany Home’s Med Tech/ Personal Caregiver position will be a front-line employee offering personal care services to the residents in our Assisted Living & Memory Care Programs.  They will be responsible to Beth Haven Management.  This will include working with the other Caregivers to insure that resident needs pertaining to the activities of daily living are met.

·  Use safety measures for resident and self when using body mechanics / no lift policy.
·  Assist with safe transfer to and from bed, chair, sofa, commode and walker.
·  Follow basic hygiene and infection control practices; a list will be reviewed and distributed to all staff.
·  Contribute to a positive, healthy, and safe living and work environment.
·  Recognize emergencies and have knowledge of emergency procedures associated with what is presented.
·  Offer companionship / social interaction too residents.
·  Observe functioning of residents.
·  Assist residents with the activities of daily living.
