Skilled Nursing


Job Title:         CNA (All Shifts Available Full Time and Part Time)
Reports To:     Director of Nursing

The Certified Nursing Assistant (CNA) will give basic bedside nursing care through learned nursing tasks to reach or maintain the resident's highest practical physical, mental and psychosocial well-being by performing the following duties:

Essential Duties and Responsibilities include the following. Other duties may be assigned.

  • Follows a daily routine for the residents in accordance with nursing care plan in areas of physical needs.

  • Carry's out procedures as assigned such as: Vital signs, temperature, (oral, axillary, rectal) pulse, respiration and blood pressure. Admission, transfer or discharge of a new resident. Intake and output, measuring, estimating and recording. Specimen collection:  urine, stool, sputum. Serve meals and feed resident if necessary. Encouraging, assisting and training in self-care and activities of daily living.

  • Follows safety measures in regard to self and resident with regards to: Body mechanics when re-positioning and transferring. Resident positioning and alignment including utilization of bed cradle, trapeze, mechanical lift, special mattresses, pillows, and wheelchair cushions. Resident transferring to wheelchair, bed, shower chair or other chair. Side rails, wheelchair and ambulation devices. Fire and Disaster Plan and unusual occurrences.

  • Report's to duty as scheduled by maintaining personal wellness and organization of personal activities to prevent a shortage of care-giving staff.  

  • Understands the plan of care for each resident in your assignment.

  • Report's any change in the resident's condition or ADLs immediately to the charge nurse.

  • Be familiar with Bethany Home policies and adhere to same.

  • Observes all residents' rights.

  • Provides visual privacy to all residents during personal care and treatments.

  • Maintains proper placement of resident's call bell and answers it promptly.

  • Leaves the unit (bed, bedside stand, over-bed table and vanity) clean and in neat order for the on-coming shift.

  • Attends 24 hours of in-service education annually, including the required in-services to meet state and federal regulations, to update, enhance and improve basic knowledge and skills. 

  • Remains on duty until properly relieved and checked out with charge nurse.

  • Carry's out other tasks as assigned by charge nurse.


Job Title:         Charge Nurse
Reports To:     Director of Nursing

Summary - The charge nurse is responsible to the Director of Nursing or designee to provide nursing services on the assigned unit to assure quality of care according to Bethany Home policies and procedures.  The licensed nurses are in charge of the facility in the absence of administrative or supervisory staff.  Their responsibilities include taking charge in an emergency of any kind and directing activities toward the safety and security of the residents according to policy and procedure in the emergency and disaster plan. 

Essential Duties and Responsibilities include the following.   Other duties may be assigned.

  •  Maintains awareness of comfort and safety needs of patient.

  • Takes temperature, pulse, blood pressure, and other vital signs to detect deviations from normal and assess condition of patient.

  • Responds to life saving situations based upon nursing standards, policies, procedures, and protocol.

  • Maintain compliance with regulations established for professional licensure and for facility licensure.

  • Attend shift report and count narcotics at the beginning and end of each shift.

  • Accurate pouring and timely administration of all medications and immediate charting of them in accordance with established procedures. 

  • Carrying out all treatment orders and changing of dressings as ordered by the resident's physician.  

  • Notify the physician according to policy and procedure.

  • Ensure that diets are served as prescribed.

  • Order medications as supply demands.  Check and sign in the medications when received in the facility from the pharmacy.

  • Monitor bowel schedule for each resident and assign bowel care to CNA's in accordance with the physician's orders.

  • Provide daily bowel list and follow-up with PM shift.

  • Monitor Hemocults to ensure compliance.

  • Monitor Meal percentages. 

  • Properly note physician's orders, receive telephone orders and handle in accordance with established policy.

  • Assure proper catheter care and change catheters according to physician's orders.

  • Maintain resident's records in accordance with established procedures, including daily charting when appropriate, weekly summaries, skin assessment, updating according to schedule, nursing care plans, and monitoring proper charting by nurse assistants.

  • Verbally communicate daily to nursing assistant’s specific treatments, special orders, and nursing care procedures to be carried out for each resident. 

  • Monitor nursing staff for implementation of residents plan for care, expected outcome and the quality of care according to Bethany Home expectations.

  • Give complete report on patients' condition, needs, orders, medication, and activity to the on-coming nursing personnel.

  • Assist in writing performance evaluations for CNAs under direct supervision.

  • Maintain clean nurses' station and medicine cart.

  • Must maintain California License in good standing. 

  • Must attend mandatory in-services and make every effort to attend monthly LN meeting. 

  • Any other tasks which might be assigned by the supervisor.


Job Title:         Medical Records Clerk (Full Time)
Reports To:     Director of Nursing

Summary: The Medical Records Clerk shall possess general clerical skills including typing, filing, and inventory control skills.  Shall possess high standards of accuracy.  The employee responsible for the medical record system is either certified or trained to perform Medical Record duties and her/his work receives regular monitoring by a Medical Records Consultant and the Medical Records Supervisor.  The Medical Records Clerk is responsible to the Medical Records Supervisor.  Under the guidance of a medical record consultant, the Medical Records Clerk maintains the record-keeping practices of the facility as established by the facility policies and procedures.

Essential Duties and Responsibilities include the following. Other duties may be assigned.
- Maintain permanent register of residents - resident index.
-   Maintain disease index/coding in Registration Program and on cummulatives.
- Review records of discharge and admission charts using the chart audit.
-   Do Admission / 72 hr. / 7-day audit on all new admits and notification of Medical Records Supervisor of any missing items.
- Re-audit # 9 monthly for completions.
-   Thin charts on a regular basis.
-    Preserve and protect medical records in your custody.
-    Handle routine requests for medical information. (In house)


Job Title:         Administrative Assistant (Part Time)

Reports To:     Skilled Nursing Facility (SNF) Administrator

Summary - This part-time position will be under the general supervision of the SNF Administrator, and serve as the primary point of internal and external contact, performs receptionist and secretarial duties, and many aspects of the admissions process.  This position is responsible for assisting the SNF Administrator in ensuring SNF operations remain in compliancy with regulations, policies and procedures.

Essential Duties and Responsibilities include the following: (Other duties may be assigned.)

·         Serves as the receptionist for the Skilled Nursing Facility.
·         Greets and acknowledges visitors. 
·         Monitors the visitor and vendor badge program.
·         Receives and screens telephone communications.
·         Maintains an organized desk area, ensuring residents’ protected health information is kept secured.
·         Maintains SNF Conference Room and Work Room in order.  Oversees schedule for Conference Room.
·         Receives and distributes incoming mail.
·         Maintains the filing system for the office including the residents’ business files.
·         Monitors the usage, ordering and receiving of office supplies for the facility.
·         Provides clerical work for the SNF Administrator and other administrative staff.
·         Appropriately answers questions from admission inquiries.  Documents and tracks admission
inquiries.  Documents reasons for refusals.
·         Provides tours of the Skilled Nursing Facility.
·         Maintains a sufficient supply of Applications and Admissions Packets, ensuring documents are

Certificates, Licenses, Registrations - Must be able to type at least 60 words per minute.

Other Qualifications: Must demonstrate a professional manner in dealing with both the general public and the residents, have exceptional communication skills, be skilled at solving problems, and be capable of generating confidence in interpersonal relationships.  A working knowledge of electronic health records and a basic understanding of business procedures are also required, together with a total commitment to the Mission Statement of Bethany Home.

Assisted Living and Memory Care Facility

Part Time AM/PM Shifts Available

Bethany Home’s Med Tech/ Personal Caregiver position will be a front-line employee offering personal care services to the residents in our Assisted Living & Memory Care Programs.  They will be responsible to Beth Haven Management.  This will include working with the other Caregivers to insure that resident needs pertaining to the activities of daily living are met.

·  Use safety measures for resident and self when using body mechanics / no lift policy.
·  Assist with safe transfer to and from bed, chair, sofa, commode and walker.
·  Follow basic hygiene and infection control practices; a list will be reviewed and distributed to all staff.
·  Contribute to a positive, healthy, and safe living and work environment.
·  Recognize emergencies and have knowledge of emergency procedures associated with what is presented.
·  Offer companionship / social interaction too residents.
·  Observe functioning of residents.
·  Assist residents with the activities of daily living.
